Proof of critical illness
To process your request as quickly as possible, we will need proof of your critical condition.
This needs to be on headed paper, signed by a medical professional and dated within the last 6-12 months.
If you are living in a care home, we also need a letter from the care home on headed paper confirming your residency and dated within the last 6-12 months.
