When completing this form, make sure the details you give match your Vodafone account. If they don’t, we might not be able to process your request. Check your details in My Vodafone here
and update them if needed.
What you need to know
We aim to get your proof to you within three working days.
We can only provide proof of purchase for a phone bought directly from us. But if you bought yours from someone else – like Carphone Warehouse – we can still provide proof of usage or that it has been reported as lost or stolen.
