If we no longer have your correct name or title, the easiest way to let us know is by using this form.  Once you're done, we'll start the correction process.  We'll aim to make the change as soon as possible.

When you've hit "Submit", we'll take it from there.  You'll get a confirmation email, and if we need any more information we'll be in touch.

We also support your application for your Gender Recognition Certificate, if you need one, through our Living As Me scheme.

The maximum size for each attachment is 2MB.
The total size for all attachments must be less than 10MB.

Please note: some devices may take pictures in a higher quality, meaning they may be over 2MB.  You'll need to reduce the size of these images before you upload them.
