You can also use this form if you’ve been made a deputy (in England and Wales) or a controller (in Northern Ireland).

You’ll need to attach a copy of:
  • Your passport or driver’s licence
  • The account holder’s passport or driver’s licence
  • A registered copy of the legal document stating the power of attorney you’ve been given
